Accurate records and continuity · NREMT EMT

8 min + practice · Original foundation lesson

Preserve what was observed, who reported it, what was done, and what changed.

Objective and boundary

Preserve what was observed, who reported it, what was done, and what changed. This lesson is an assessment foundation. It does not authorize an intervention or replace a clinical protocol. State what is known, what is reported, and what remains uncertain before selecting a response.

Distinctions that change the decision

Document findings accurately: A record should preserve the assessed finding rather than a convenient invented value. Witness information must not be represented as the clinician direct observation. The record should describe actual interventions and what followed. Corrections should preserve record integrity under the documentation policy. Choose an objective documentation style: Observable behavior can be assessed more reliably than a judgmental label. Patient statements should remain identified as statements. Numeric findings require units and timing for interpretation. Motive claims require evidence and are often inappropriate substitutes for findings. Recognize missing handoff information: The receiving team must know which patient the report describes. A single current value may hide deterioration or improvement. The details of an intervention affect continuing assessment. A handoff process should make responsibility transfer clear.

Worked contrasts

The crew did not obtain a blood pressure. Which record principle applies instead of entering a typical normal value? The answer is Record what was observed or measured. An unmeasured value must not be invented. A record should preserve the assessed finding rather than a convenient invented value. A coworker reports a seizure that the crew did not see. How should that event be documented? The answer is Attribute a reported event to its source. The account should identify the reporting source. Witness information must not be represented as the clinician direct observation. Instead of “uncooperative,” a report says the patient turned away and declined two questions. What improved? The answer is Describe observable behavior. The report states observable actions. Observable behavior can be assessed more reliably than a judgmental label. The report includes “the room feels as if it is spinning,” identified as the patient statement. What improved? The answer is Use the patient words with attribution. The symptom description is attributed to its actual source. Patient statements should remain identified as statements.

Transfer and self-check

The crew departs without confirming who accepted the patient report. What handoff element is unclear? Pause and select a category before reading the check. Check: Missing receiving acknowledgement. Transfer of responsibility was not made explicit. Explain why one alternative might sound reasonable but answers a different question. Then identify what new finding would make you return to a primary assessment.

Avoid premature closure

A category is a tool for organizing evidence, not a final diagnosis. Recheck the patient and the scene when conditions change. A stable earlier measurement, reassuring dispatch label, or completed checklist must not override new concerning findings. Preserve an unavailable history as unknown, and seek the appropriate resource when a decision exceeds training or authority.

WORKED EXAMPLE

The crew did not obtain a blood pressure. Which record principle applies instead of entering a typical normal value?

  1. An unmeasured value must not be invented.
  2. A record should preserve the assessed finding rather than a convenient invented value.
  3. The best match is Record what was observed or measured. The alternatives describe different findings, information needs, or stages; the specific details in this case determine the choice.
Answer: Record what was observed or measured
Compare the other answer choices
Attribute a reported event to its source

Witness information must not be represented as the clinician direct observation. This is not the category selected by the stated finding: An unmeasured value must not be invented.

Record actual care and response

The record should describe actual interventions and what followed. This is not the category selected by the stated finding: An unmeasured value must not be invented.

Correct an error through the approved record process

Corrections should preserve record integrity under the documentation policy. This is not the category selected by the stated finding: An unmeasured value must not be invented.

Try it, then explain it.

Complete a short set without looking at the example. After each answer, explain which evidence or rule supports your decision and why the alternatives do not fit.
